Description 1.1.1 | Vision Statement This plan envisions a future where digital equity connects all Minnesota residents to opportunities, options, and each other. 1.1.2 | Goals and Objectives The following list provides a summary of Minnesota’s digital opportunity goals and objectives as proposed in this plan. Further details, including specific strategies and key performance indicators, are spelled out at length in Section 3.0. Goal 1: Connect People to People 1. Minnesota’s digital opportunity advocates and educators statewide are a strong, united group who can lean on each other for fresh ideas, new strategies, and consistent support. 2. All Minnesotans have access to a trusted provider of digital skills training, including training that addresses cybersecurity. 3. All Minnesotans have access to a trusted provider of quality technical support. Goal 2: Connect People to Information 1. Minnesotans can access comprehensive data and mapping tools to evaluate digital opportunity in their area as well as statewide. 2. All Minnesota cities, counties, and tribes have the opportunity to create localized data-driven digital opportunity plans to support their residents and tribal members. 3. All city, county, and tribal government units have the opportunity to re-design their websites so that they are fully accessible to people with disabilities and people with limited English literacy skills. Goal 3: Connect People to Resources 1. All Minnesota households have the option to afford the internet service available at their location. 2. All Minnesota adults have the option to have afford a large-screen device or smartphone, whichever most efficiently helps them access the applications they require. 3. New digital opportunity pathways reach Minnesotans who are at high risk for being digitally excluded.

Age-Friendly Olmsted County (AFOC) Plan

Description Age-Friendly Olmsted County (AFOC) is a cross-sector initiative to ensure that residents of Olmsted County have what they need to live with autonomy and dignity in all life’s stages. When Olmsted County became a member of the AARP Network of Age-Friendly States and Communities in early 2020, it officially joined a global movement, with roots in the World Health Organization, to help cities and communities respond to an aging population and prepare for a permanently older society.

Building Out Community Mobilization Resources Coalition

Patient Family Advisory Council Integration

Description Rochester-Area hospitals, in particular, Mayo Clinic, are running multiple Patient Family Advisory Counts in order to have a clear picture of the health care process from the view point of the patients. It would be useful for these various efforts to share their best practices, lessons learned and knowledge.

Common Good RSVP

Description Common Good RSVP matches individuals 55 years and over with local and national organizations, giving you countless opportunities to engage in your community through volunteer service. As a part of Senior Corps, RSVP is America’s largest volunteer network for people 55 and over, with more than 400,000 volunteers tackling tough issues in communities nationwide. It is operated under the direction of the Corporation for National and Community Service, a federal agency.

Community Health Assessment Process

Description The Community Health Assessment Process is about improving the health and well-being of residents in Olmsted County. Every three years the community conducts a health needs assessment to determine Olmsted County’s leading health priorities; formulate a plan to address the needs; and publish an annual progress report. Olmsted County Public Health Services, Mayo Clinic, and Olmsted Medical Center engage with diverse partners across our community to lead this process.

Construction of 125 Live

Description "Whether you are a youngster who wants to learn how to swim and skate, or somebody like me who wants to learn how to use all of the apps on my iPad, or learn how to use a new watch, or just come to meet friends, this will be the place to come," said Mark Cairns the Board President of the Rochester Senior Center. The new location will feature a 51,000 square foot facility. "There's hardly a space that's been designed that won't be used throughout the day for physical activity," said Paul Widman Director of Park & Recreation. The Construction that began in early May will feature a wide array of workout facilities, including a pool and sauna. The additions piggy-back on an even larger plan.

Learning Is Forever (LIFE)

Description As part of the Rochester Community and Technical College outreach effort, Learning Is ForEver (LIFE) provides stimulating educational programs and encourages social and personal growth for life-long learners in our growing and diverse community. This is accomplished through a variety of affordable courses including lectures, discussion groups, day trips, and other learning opportunities in an accessible setting.

Living Well with Chronic Conditions

Description Gain skills to manage their condition by valuing the power of small steps and having the support and understanding of the group. Conditions include: arthritis, high blood pressure, anxiety, diabetes, heart disease or back pain.

Population Health Science Program

Description The Population Health Science Program in the Mayo Clinic Robert D. and Patricia E. Kern Center for the Science of Health Care Delivery seeks to improve primary care practice and advance population health science through health care delivery research. The program's interdisciplinary scientific team is comprised of experts from primary care medicine, health services research, biostatistics, epidemiology, social and behavioral science, and public health. Led by clinical and research faculty members, the Population Health Science Program is grounded in epidemiological, health services and behavioral research. Program researchers are focused on: Describing disease burden and relevant health outcomes among target populations Identifying and characterizing the determinants of health outcomes in populations Improving the effectiveness and efficiency of health care delivery in primary care Evaluating the impact of primary care on patient behavior, access to care and use of services Evaluating the impact of community-based interventions on population health

Research for community education course on Advance Directives

Description Interested in researching one or more of the following medical interventions: CPR, ventilators, dialysis, tube feeding and IV hydration? The purpose is to describe these interventions in lay person’s terms, discuss potential benefits and risks, and provide statistics about survival rates. If possible, you can join me in presenting this information to community education classes about choosing a health care agent and filling out an advance directive. Class dates are October 7, 15, and 28.
